Must be 18 or older to apply. The winner of the free gastric bypass surgery will be someone with type 2
diabetes. All applicants will need to be interviewed at Forest Park Medical Center to be eligible to win.
Winner must consent to possible media coverage. From all entries, five candidates will be selected as
finalists. Final winner will be chosen by Dr. Wade Barker and must meet screening criteria for a gastric
bypass surgery. Winner will be announced in the weeks following the ADA StepOut Walk to Fight
Diabetes, which will be held on Saturday, October 31, 2009, at Rangers Ballpark in Arlington.
